Day Coaches, Volunteers and Participants,
The
OMHA has announced that all players born in 2009 and earlier including Team Officials
(Coaches, Trainers and Managers), Game Officials, Association Officials (board
members and volunteers), and third party instructors are required to be fully
vaccinated by 31 October 2021 or earlier.
CDMHA is requesting that you bring proof of vaccination to the tryouts and evaluations in order for the association to get a head start on confirming that all participants are fully vaccinated by the deadline.
Proof of vaccination will be visually verified and then logged for verification purposes.
