Provincial Vaccine Requirements for 12-17 Year Old - IMPORTANT UPDATE

As you may be aware, the province announced last week they will be removing the exemption to show proof of vaccination for children 12-17 years old participating in organized sport effective Monday, December 20.      

Therefore, County staff will be required to check for proof of vaccination (2 doses + 14 days) and personal identification for all participants, including those previously exempt or under vaccine mandates from their governing bodies starting December 20. This includes players, coaches, referees and hockey administration staff. Please be sure this information is shared with everyone impacted, including your participant families, affiliate leagues and governing bodies as deemed necessary to ensure everyone attending (including opposing teams) are aware of this new provincial requirement.  

Acceptable forms of identification for youth 12-17 years of age:

Photo identification is not required, but identification must include their name and date of birth. 

 

  • Birth certificate
  • Citizenship card
  • Driver’s license
  • Government (Ontario or other) issued identification card, including health card
  • Indian Status Card or Indigenous membership card
  • Passport
  • Permanent resident card
  • Documentation from an institution or public organization that includes the name of the holder and date of birth
